Population Health Management
Delta has invested in a culture of wellness to manage employee health. Delta’s Flight Plan merges corporate goals with a comprehensive health and well-being strategy for 85,000 employees in 287 locations.
Population health management use had measurable results – health assessment completion increased to 60 percent, biometric screening reached 39 percent, and there was a 10 percent reduction in ER visits.
Health Care Operations, Pharmacy Care Services
Dignity Health uses Optum data and analytics for revenue cycle management and medical record documentation. They also partner with OptumRx on pharmacy care services for Dignity employees.
Optum Physician Advisory Services also redesigned Dignity’s case referral guidelines and workflow processes. The model drives second-level review standardization across 40 hospitals – cases requiring review are routed for status recommendations by physician advisors.
Data and Analytics, Pharmacy Care Services
Optum has partnered with Merck to utilize data and analytics and pharmacy care services for a Learning Laboratory, using real-world data to simulate the performance of reimbursement models so payment for prescription drugs is aligned closely with health outcomes.
Together, we’re exploring the potential of outcome-based risk sharing agreements for broad adoption among payers, PBMs and pharmaceutical companies. This will help to reduce clinical and financial uncertainty with respect to prescription drug payment.
Michigan Department of Health and Human Services
Data and Analytics
The Michigan Department of Health and Human Services used Optum data and analytics and pharmacy care services for CareConnect360, a Medicaid care management tool for case workers to understand, analyze and monitor care received by foster children.
Soon, more than 1,800 foster care workers and supervisors from partner agencies will have access to the tool, allowing them to review a child’s medical history before a home visit and proactively engage foster parents about their child’s needs.
